Submit this form after a qualifying tournament finish to request contingency payout review.

Before you submit

Fields marked * are required (including tournament trail, results document, installation photo, and confirmations).
Your email must match an approved contingency registration before a claim can be submitted.
Connect Stripe Express (bank deposit) before submitting — claims cannot be submitted until Stripe is connected.
Required uploads: PDF, JPG, PNG, WEBP, or GIF — max 15 MB each. Files upload directly to secure storage (not through the form server).
Prefer clear photos or PDFs. Files over 15 MB cannot be uploaded. On iPhone, use a lower-resolution photo or compress before uploading.
